Application Instructions
Dear Applicant:
Thank you for your interest in working for our company. Please print out the application and criminal background request form. See below for instructions on how to fill out the forms.
Application:
- Completely fill in all four pages of the application
- On page four make sure all information is filled in accurately; this is the form we will use to verify your reference
Our Staff Requirements
- C.N.A.Certified Nurses Assistant Certified
- Criminal Background Checks
- Qualified References
- Yearly TB Screening
- Continuing Education Classes
Criminal Background Request Form:
- Fill in the blanks with a star beside them
- Do not cross out on the form; if there is anything crossed out they will not run the background
- Attach or fax a copy of your current driver's license (ensure birth date and license number are clear). We cannot run your background or process your application without a copy of your license.
Once you have filled out these forms either fax to 770-552-9502 or email the forms to the Office Manager.
Once you have done this please call 770-643-1712 to make sure we have received your paperwork. We will then run a background and reference check. As long as both of these checks come back clear we will set-up a time for you to come into our office and do an interview.
